Student Performance Evaluation Form (PDF)
Complete the student performance evaluation once you get to know your co-op student. If your organization has its own employee evaluation form, the Co-op Office will also accept this documentation. Three copies are needed one for the Co-op Office, your Co-op Student, and one for your own records.
